Job Overview

The Rehabilitation Care Assistant supports the rehabilitation and therapy team in delivering safe, person-centred rehabilitation and care interventions for clients and residents across Intermediate and Long-Term Care (ILTC), community care and residential care settings.

Working under the direction and supervision of qualified therapists and healthcare professionals, the Rehabilitation Care Assistant facilitates clients’ participation in rehabilitation activities, promotes functional independence and supports continuity of care to enhance their overall quality of life.

Key Responsibilities

• Carry out rehabilitation and therapy interventions under the supervision and guidance of Therapists.
• Assist in conducting individual and group therapy programmes for clients and residents.
• Support the planning and delivery of recreational, wellness and functional maintenance programmes for elderly clients.
• Assist clients with basic care needs during therapy and programme sessions.
• Prepare therapy areas, equipment and treatment materials for rehabilitation activities.
• Monitor and ensure the safe use, maintenance and inventory control of therapy equipment and supplies.
• Perform administrative duties, including documentation, appointment coordination and data entry as required.
• Support therapists in monitoring clients’ participation and progress during rehabilitation programmes.
• Act as an effective member of the multidisciplinary rehabilitation team.
• Demonstrate positive interpersonal and communication skills when interacting with clients, caregivers, colleagues and members of the public.

Requirements

• Certificate, WSQ, NITEC, Higher NITEC or equivalent qualification in Healthcare, Community Care, Therapy Assistance, Rehabilitation, Nursing or a related discipline.
• On-the-job training will be provided.
• Demonstrates patience, empathy and a genuine willingness to serve and care for elderly clients.
• Team player with good interpersonal, communication and organisational skills.
• Ability to work independently and manage tasks effectively within established timelines.
• Physically fit and able to assist clients with mobility and rehabilitation activities.
• Passionate about supporting clients in achieving greater independence and well-being.

Working Arrangement

• This role may require staggered working hours and occasional weekend or public holiday duties to support centre operations, programmes and events.
• Opportunities may be available to gain exposure across different community care and residential care settings.
