Guide To Requesting A Medical Report
Obtaining Your Medical Report
Requests for medical reports are applicable only for patients of Kwong Wai Shiu Hospital (KWSH). A medical report is prepared by a doctor based on a factual record of the patient’s medical condition.
To apply for your medical report, you must submit written consent for the release of your medical information.
In the case of a deceased patient or a minor, the written consent of their authorised representative or immediate next-of-kin must be obtained. The requester is not allowed to assign a third party to collect the report.

How Do I Submit a Request for a Medical Report?
Option 1: Download and Submit Forms Manually
If you are the Patient, at least 21 years old, please complete Form A – Application and consent for release of medical report.
If you need a representative to collect the medical report on your behalf, you are required to complete Form C – Authorisation for collection of medical report form. Your representative must self-collect from our officer by presenting this form on the day of collection.
If you are applying for a patient who has deceased, please complete Form B – Application and consent for release of medical report on behalf of patient.
Please ensure all fields are completed and filled out correctly to avoid delays in processing.
We can only process your application upon fulfilling the verification and receipt of all necessary forms, supporting documents and payment.

Medical Report Fee
Ordinary Medical Report – $163.50 including GST.
* Charges listed above are inclusive of prevailing GST and subject to change without prior notice.
Payment Methods
We accept payment via PayNow. Scan the PayNow QR quote in the form and indicate “MRO-[insert surname of applicant]” as your Bill Reference number, eg., “MRO-Lim”.

What are the Supporting Documents Required?
Forms and supporting documents required are:
- Copy of the completed, Form A – Application and Consent for Release of Medical Information/Report”, or, “Form B – Application and consent for release of medical information on behalf of patient”.
- Scanned copies / photocopies of the patient’s NRIC (or appropriate identification documents), both front and back views.
- Scanned copies / photocopies of the applicant’s NRIC (or appropriate identification documents), both front and back views..
- Scanned copies / photocopies of relevant documents (e.g. birth certificate, marriage certificate and letters of administration) are to be attached as proof of relationship to the patient..

Cancellation Policy
There will be no refund provided upon cancellation.
The release of the medical report is subjected to official approval by KWSH.
As a general guide, the time required for processing medical reports is about three weeks from the date of receiving the completed forms, or the date of medical appointment for assessment, whichever comes later.
