We aim to provide holistic community and home healthcare services for the homebound elderly and caregiver training to support caregivers in taking care of their elderly at home.

In collaboration with other home care service providers, we provide the following Home Care Services:
  • Home Medical Service
  • Home Nursing Service
  • Home Therapy Service
  • Home Personal Care Service

Caregiving Training

As an approved Caregiver Training centre, we provide the following Caregiver Trainings:
  • Basic Skills in Caregiving (for caregivers and foreign domestic workers):
    • Classroom Based (7 hours)
    • Home Based (3 hours)
  • Modular Classes:
    • Classroom Based (2.5 hours)
    • Home Based (2 hours)

Home Care Services

The objective of our Home Care Services is to provide affordable and holistic person-centred home healthcare services to homebound elderly so that they can age in place and remain in the community as long as possible.

Some elderly are homebound due to reasons such as their impaired mobility, chronic medical conditions, wheelchair accessibility issues, mental conditions, etc. To help these elderly who may have different needs, Kwong Wai Shiu Hospital’s Community Services provides the following Home Care Services:

  1. Home Medical Service
  2. Home Nursing Service
  3. Home Therapy Service
  4. Home Personal Care Service

Our Home Care elderly can access subsidised medication in polyclinics and subsidised purchase of consumables and assistive devices under the MOH’s Medication Subsidy Programme and Senior Mobility Fund respectively if they meet the criteria. 

Our doctor will conduct home visit to those elderly who are not able to visit the doctor regularly. Our home medical services include:

  • Geriatrician-guided assessment of more complex cases 
  • Management of chronic medical problems
  • Management of uncomplicated acute or sub-acute medical problems
  • Referral of patients to specialists in other disciplines, where appropriate  
  • Urgent prescription of medication available in retail pharmacies or polyclinics
  • Educating the patient and caregiver on the patient’s illnesses and the management plan 
  • Working in collaboration with other primary health care providers, where necessary 
 

The Home Care team assists the frail, homebound or bedridden elderly who require the following nursing care:

  • Post-surgical management – administration of injections, care of central venous line, tracheostomy or drainage tubes.
  • Wound management
  • Maintenance of urinary catheters and drainage tubes
  • Stoma care – colostomy and ileostomy care
  • Pain control
  • Insertion of nasogastric tube and tube feeding.
  • Assistance with bowel elimination – enema or manual evacuation
  • Monitoring of client’s medical condition e.g. blood pressure and blood sugar checks
  • Medication management
  • Provision of caregiver education
 

The Home Care team assists the homebound elderly with rehabilitation to improve/maintain their activities of daily living like personal grooming, feeding, mobility, etc.

Our therapists will assist in: 

  • Conducting falls prevention assessment at home and recommending modifications with the aim of improving safety and independence for the patient, and reducing caregiver stress.
  • Providing caregiver education with regard to knowledge of disability and the importance of rehabilitation, prevention of further disability, proper transferring techniques and maintenance rehabilitation at home, eg. stretching, range of motion exercises and assistance in activities of daily living.
  • Improving sitting balance for bed-bound patients
  • Training in mobility at home
 
The Home Therapy Service covers physiotherapy, occupational therapy and environmental assessment.
 

The Home Care team provides personal homecare services that are non-medical to the elderly, who experience impaired ability to perform the activities of daily living and require assistance to maintain their independence and well-being to remain in a safe, clean and healthy home environment. These services are responsive to the distinct needs of the elderly and community and are delivered by qualified home care workers.

Our Home Personal Care Service can be classified as follows:

  • Personal Care - includes assistance in activities of daily living (ADL), such as bathing, dressing, toileting, grooming, mobility assistance, incontinence care and feeding.
  • Homemaking - includes assistance with simple housecleaning, laundry and meal preparation. 
  • Mind Stimulating Activities - include games and activities.
  • Activities for Physical Well-being - include simple physical exercise programme.
  • Medication Reminder Service – includes monitoring client’s medication compliance and assisting with administration of medication according to guidelines.
  • Other Personal Care Tasks – like monitoring of vital signs, specimen collection, and dressing for stable surface wounds.
 
Those aged above 55 years old and are homebound or frail with chronic diseases under the following categories:
  • Category II
    • Ambulant with up to 1 person minimal assistance +/- walking aids and/or mildly demented.
    • Require some supervision in ADL, such as transfer, feeding (may require tube feeding), toileting and personal grooming.
  • Category III
    • Wheel-chair bound or ambulant with 1 or 2 persons’ moderate assistance onwards +/- walking aids.
    • Needs supervision most of the time and require help in ADL, such as mobility, transfer, feeding / tube feeding, diaper / urinary catheter, and simple wound dressing.
  • Category IV
    • Bedridden
    • Require total assistance and supervision for every aspect of ADL
** Patients of other age groups who are in Category II to IV may be considered on a case-by-case basis.

We are currently providing Home Care Services around KWSH area in Whampoa and Kallang region. We also cover other areas like Toa Payoh, Mountbatten, Hougang and Serangoon. However, we can consider serving elderly beyond these areas on a case by case basis. 

 

Service users are charged according to Household per Capita Monthly Income and National Means Test System (NMTS) subsidy level. Patients with a monthly per capita household income of S$2,600 and below can undergo means testing to be eligible for fee subsidy. If not, full paying rate is chargeable.

Caregiver Training

Are you caring for a loved one who is having or recovering from a medical condition, or one with disabilities? Caring for a loved one can be an emotionally and physically trying experience though it is also extremely rewarding. Being a caregiver involves many responsibilities that may be new to someone who has just become a caregiver. It could be demanding and stressful, especially when you are not sure of what you are doing. Hence, it will be empowering and valuable to receive proper caregiver training. You’ll feel better when you are confident of your skills and we are here to help!

The Caregivers Training Grant, or CTG for short, provides families and Caregiver with a subsidy when they apply for training that will equip them to better care for the physical and socio-emotional needs of persons with disabilities or the frail, homebound elderly.

The CTG allows caregiver to tap on a $200 annual subsidy to attend CTG-approved training courses. The grant is administered by the Agency for Integrated Care (AIC), and given out in the form of training fee subsidy for caregivers.

The amount of $200 is allocated to each care recipient every financial year (starting from Apr to Mar the following year). For example, if there are two caregivers caring for one care recipient, the total grant eligible is $200 per year and not $400, as there is only one care recipient.

Our caregiver training is open to all adults who are caregivers or preparing to be one. Those who are not eligible for the CTG will have to pay the full course fee.

To be eligible for the grant, care recipients must be Singaporeans or Permanent Residents above the age of 65 years or have a disability (as certified by a Singapore-registered doctor).  Family members or foreign domestic workers who are the main caregivers can attend a wide range of CTG-approved courses conducted by various agencies.

The grant can only be used to subsidise a list of training courses that have been pre-approved by AIC, the administrator of the grant.
