乐龄护理中心
2016年10月1日，为了配合政府所推广的以原地养老应对人口老龄化的计划，日间康复中心转型并更名为“乐龄护理中心”（Senior Care Centre, SCC）。乐龄护理中心整合了原有的康复与复健服务，进而提供一系列的护理服务以协助年长者提升或维持他们的身心健康，从而达到在地老龄化的目标。
我们的服务
我们的专业医疗团队包括职能治疗师、物理治疗师、语言治疗师及注册护士。
专业服务
我们的治疗师通过复健治疗协助病患发挥最大的复健潜能及提升身体机能，帮助他们在社区中安乐地生活。
职能治疗
职能治疗师协助病患提升他们的体能、认知及社会心理健康，以促进他们在社区里独立生活的能力。
物理治疗
物理治疗师协助病患提升或维持他们的身体功能，以及延缓因疾病或老化所带来的失能。
Physiotherapy
To maintain and/or maximise our client’s functional ability and also delay rate of physical deterioration due to diseases and ageing.
Speech Therapy
To provide diagnosis and management of swallowing disorders, and a range of communication disorders including language, speech and voice disorders.
语言治疗
语言治疗师提供吞咽困难及沟通障碍（包括语言及发音相关障碍）相关的诊断及治疗。
医疗护理服务
提供定期的健康检查、伤口管理、药物管理及插管等服务。
自2016年10月1日起所增添的新服务配套
我们提供以下服务：
星期日托管护理
星期日托管护理为乐龄人士提供看管照护服务，满足年长者基本需求的同时，也让他们的家人和看护者得到暂时的缓解。这项服务可以让家人和看护者得到休息或好好利用时间处理个人事务，有助于减轻他们的压力并避免倦怠感。乐龄人士则可得益于专业的护理服务，包括基本的监督和日常活动辅助。看护者和年长人士都将从中受益。
社区康复治疗
社区康复通过完整的复健计划协助病患重新融入社区。我们的治疗师主要通过一对一的治疗方式协助提升或维持病患的身体功能，以及延缓因疾病或老化所带来的失能，并且达到在社区中安乐生活的目标。
维护健康运动
维护运动主要通过小组活动，提供社交及休闲活动、认知活动、肌力及平衡训练，以协助我们的病患维持他们的身体功能及延缓因疾病或老化所带来的失能。
老人日托服务
本中心提供日间托管服务，让年长者有机会参与治疗师所设计的体能活动及社交活动（例如：团体外出、休闲活动及认知活动）。老人日托服务也提供膳食。
失智症日间关护
本中心提供一系列完整且针对失智症不同阶段的关护服务。活动主要以小组形式进行，包括提升定向感、认知刺激疗法、怀旧活动。针对病情较为严重的晚期失智症患者，日间看护服务也以非药物管理方式应对他们的行为症状。失智症日间关护服务将提供膳食。 本中心也为关护者提供培训并给予他们指导。
社区护理服务
提供健康检查、伤口管理、药物管理及鼻胃管插管等服务。
电动轮椅评估及训练
广惠肇留医院乐龄护理中心自2015年6月以来成为护联中心（Agency for Integrated Care, AIC）所指定的电动轮椅评估及训练中心。此评估及训练可确保年长者行动基金（Senior Mobility Fund, SMF）的申请者懂得安全的使用电动轮椅。乐龄护理中心设有适当的设施与配备，由本院合格的职能治疗师进行评估并提供训练。

接送服务
乐龄护理中心为居住在广惠肇留医院3公里以内的年长者提供来回接送服务。
申请使用乐龄护理中心服务
有意申请使用乐龄护理中心服务的年长者需获得医生（指新加坡医药理事会的注册医生）认证，证明他们健康情况稳定并适合进行康复疗程。申请者也必须接受家庭经济状况调查。
使用乐龄护理中心服务者的常见情况：
- 跌倒高风险群或有经常跌倒的情况
- 行动不便，例如：行走或出入不便
- 需要他人照顾生活起居
- 认知及知觉障碍，例如：失忆与记忆力不佳
- 脑神经相关病况，例如：中风、帕金森症
- 骨科相关伤病
- 肌肉骨骼相关病况，例如：骨关节炎、肌肉损伤
- 基本与日常生活功能衰退
- 失智症

星期一至五：上午7点至晚上7点

All Day Care Services include lunch and 2 tea-breaks.
A refundable deposit of S$200 is required for all rehabilitation services.
Basic ward charges include accommodation, meals, standard medication & procedures, and laundry.
A deposit of S$1,380 is required for all nursing home services. The deposit paid on admission is refundable. However, it will be used to offset any unsettled invoice first. The above rates do not include other inpatient variable charges (if applicable).

Transportation
2-way transportation will be provided to clients staying within a 3 km radius of KWSH.
Eligibility Criteria
Clients with stable medical conditions and who wish to enrol in our Senior Care Centre rehabilitation services have to be certified and deemed fit for rehabilitation by doctors registered under the Singapore Medical Council. It is also compulsory for them to undergo means testing.
Conditions include but not limited to:
- Increase fall risk or recurrent falls.
- Mobility issues such as difficulties in walking and transferring.
- Requires assistance to care for self.
- Affected cognitive perceptual abilities, e.g. early loss of memory.
- Neuro-related problems such as Stroke Disease, Parkinson’s Disease and etc.
- Post-operation medical or orthopaedic conditions.
- Musculoskeletal/Orthopaedic conditions e.g. osteoarthritis, muscle pain.
- Functional decline and deconditioning.
- Dementia.
